Who makes the referral?

You can actually refer yourself! A referral can also be made by a family member, physician or other provider who is familiar with your condition. To make a referral contact your local PACE Center.

To enroll in a PACE program, you must meet the following eligibility requirements:

• age 55 or older
• reside in the PACE organization’s service area
• be certified as eligible for nursing home care in NC and
• be able to live safely in a community setting at the time of enrollment.
